Strength Training Programs for Beginners to Elite Athletes

I am interested in how different teams manage their substitutes that get no, or minimal playing time (perhaps just the last 5-10 minutes of a game), so as these players can maintain their conditioning despite the lack of game time?
Do teams work these players out post game or the next day?
And what are the format, aims and methods teams use?
All ideas gratefully received.
Any ideas out there?
Depending on our schedule we work the subs out either straight after a game or the day after.
Also our subs are governed by injuries or tactical changes, not conditioning.
